What does P-01 Tian Chi do?

P-01 unbinds the chest, transforms phlegm and descends rebellion, regulates qi and dissipates nodules, and benefits the breasts — and it is both a Window of Heaven point and a meeting point of four channels.

Two classifications at once, both worth stating. Our record gives P-01 as a Meeting Point of the Pericardium, Gallbladder, Liver and San Jiao channels — four pathways — and as a Window of Heaven point. It is the first point of the Pericardium channel, on the chest beside the breast, and the convergence of four channels there is what its multi-region indications reflect.

The one Window of Heaven point on the chest. The ten Window of Heaven points are BL-10, DU-16, LI-18, LU-03, P-01, REN-22, SI-16, SI-17, SJ-16 and ST-09 — eight on the neck and head, and two off it: LU-03 on the arm and P-01 here on the chest. Our LU-03 record sets out that the two arm-and-chest points are the exceptions to a group that otherwise sits on the neck. That both the outliers are Lung- and Pericardium-channel points — the two channels most tied to the chest and the Heart — is consistent with the group’s classical role governing qi between the trunk and the head.

On the name. “Tian Chi” means Celestial Pool — tian heaven, marking the Window of Heaven role, and chi a pool or pond. The tian here genuinely marks the classification, unlike the tian traps at BL-07, SJ-15 and GB-09 this project has documented.

What is P-01 Tian Chi used for?

P-01 is used for a suffocating sensation in the chest, pain in the flank, and swelling and pain in the armpit region. Our own indication record is reproduced here in full.

The axillary swelling is the “dissipates nodules” action, and it belongs to this location. The point sits near the axillary lymph nodes, and swelling of that region — along with the scrofula and lump indications the classical texts attach to it — reflects its position. Chest oppression and flank pain are the Pericardium’s and the meeting channels’ chest territory.

An honest note, and two entries need one. A SUFFOCATING SENSATION IN THE CHEST, OR CHEST TIGHTNESS WITH BREATHLESSNESS, SWEATING, OR PAIN SPREADING TO THE ARM OR JAW, IS A MEDICAL EMERGENCY — a point recorded for chest oppression sits directly over the heart and lung, and that combination requires emergency care. ANY PERSISTENT SWELLING OR LUMP IN THE ARMPIT OR BREAST REQUIRES MEDICAL ASSESSMENT — the causes range from infection to lymphoma to breast cancer, and a lump near the breast or in the axilla must be examined, not watched. This record documents classical use only and is not a substitute for assessment of a lump or of chest symptoms.

How it is combined. With P-02 Tian Quan below on the upper arm, and P-06 Nei Guan distally — the channel’s most-used point, for the chest. With REN-17 Shan Zhong for the chest and breasts, and GB-21 Jian Jing for the axillary and breast region. All held here.

Where is P-01 Tian Chi located?

P-01 is on the chest, one cun to the outer side of the nipple and slightly above it, in the fourth space between the ribs.

• ST-17 Ru Zhong is the nipple itself, the landmark it is measured from

Measured from the nipple — with the same caveat that landmark always carries. Our record locates P-01 one cun lateral to the nipple, and our ST-17 record sets out that the nipple is a reliable reference only in the young male body; in women and older men it descends and moves, and the fourth intercostal space must then be found by counting ribs from the angle of Louis. Because P-01 sits beside the breast, that caveat matters especially here, and the rib count is the method to use where the nipple has shifted.

On the name. “Tian Chi” — Celestial Pool, the tian marking its Window of Heaven role.

How is P-01 Tian Chi needled?

Our own record states OBLIQUE insertion of 0.2–0.4 inch, with the explicit note that DEEP PUNCTURE IS NOT ADVISABLE, and moxibustion applicable — because this point sits on the chest with the lung beneath it. What follows is a neutral record of what the reference literature documents. It is not instruction, and this page does not teach technique.

• Angle and depth as recorded: oblique, 0.2–0.4 inch — among the shallowest recorded depths on the chest, and the obliquity keeps the needle in the chest wall.
• Our record states: deep puncture is not advisable. Few chest points carry a depth prohibition in their own words; where one exists this library retains it verbatim.
• Moxibustion: recorded as applicable.
• Regional anatomy: pectoralis major and pectoralis minor overlie the intercostal muscles; the intercostal nerve and vessels run along the lower border of the rib above; the pleura and lung lie deep to the intercostal muscles; breast tissue overlies the point superficially, more extensively in women.

Pneumothorax is the reason for the shallow oblique angle and the prohibition. This is a point on the chest over the lung, and the recorded caution addresses exactly that. Our LU-01 and LU-02 records carry an explicit lung-injury caution for the same reason, being over the lung apex a short distance up — and P-01 is the Pericardium-channel point of the same band. The breast tissue overlying the point is a further reason for the shallow angle.

What is not claimed. No study measuring a safe depth at P-01 could be located, and none is borrowed. The paediatric chest-CT study this library uses measured 23 points on the back and shoulder, not the front of the chest. No case report naming P-01 was located.

Is there research on P-01, and what should a reader know?

No trial isolates P-01. Where the point appears it is one component of a chest or breast protocol, and a study that includes a point in a protocol is not evidence about that point.

The channel’s evidence belongs to P-06 Nei Guan, further down the arm — the one acupuncture point with a genuinely strong Western evidence base, as our P-04, P-06 and P-07 records set out. Nothing from that literature is about P-01.

What this page can state with confidence. P-01 is the first point of the Pericardium channel, and it carries two classifications: a meeting point of four channels, and a Window of Heaven point — one of only two of that group off the neck and head, with LU-03. Its record carries an explicit “deep puncture is not advisable” because it sits on the chest over the lung, beside the breast. And its tian genuinely marks its classification, which this project has learned not to assume.

The chest and lump warnings are the operative content. Chest oppression with breathlessness is an emergency, and a lump in the breast or armpit needs assessment.

A note on searching for it. “PC 1”, “P 1” and “Tianchi” searches return protocol trials and a very large volume of unrelated material — “Tianchi” is a famous crater lake and a common place name, which dominates results, and our library codes this channel P- while much of the literature uses PC-. Any claim attached to this point should be checked against what a study actually used.
